  Abstract

  Background: Drug-Related Problems (DRPs) account for $76.6 billion in hospital costs, 8.7 million hospital admissions, and 17 million emergency room visits annually. Identification and resolution of DRPs predominantly help to achieve better therapeutic outcomes and improved patient care. Objective: The study aimed to analyse the impact of past medications on current admission in a tertiary care hospital. Methods: A hospital-based prospective observational study was conducted over a period of six months from October 2019 to March 2020. The necessary data was collected from in-patient case notes, past treatment charts, interviews with patients or patient caregivers and nursing staff.
